Assertive Outreach Team: A multi-disciplinary team engages with individuals living with mental illnesses, and who are living in shelters or are homeless. We help individuals connect to income sources, health care, housing and other support services.
Case Management: Public Health Nurses work with adults who are who are diagnosed as living with a serious mental illness to promote the achievement of optimal health and well-being and to increase independence and participation in community life.
