Coordination des services et prise en charge
La gestion de cas est un service où un gestionnaire de cas rencontre le client (un individu ou une famille), afin de déterminer quels services sont nécessaires. Ensuite, le gestionnaire de cas organise les services et les mécanismes de soutien dans la communauté. La coordination de services sert à coordonner les différents services de manière efficace afin de mieux aider la personne.
